(1) - This form has all the policies for Indian Creek High School Athletics, as well as the Certificate of Waiver and Pledge Sheet that are required for all athletes before you are able to participate in a game/event.

(2) - This for is required for all Indian Creek High School athletes. It must be signed by the Athlete and Parent/Guardian and turned in before you are able to participate in a game/event.

(3) - This is a list of all banned substances by the IHSA.

(4) - This form is has all the policies for Indian Creek Midele School Athetics, as weel as the Certificate of Waiver. Both pages are required to be signed and turned in before you are able to participate in a game/event.

(5) - This form is information on requirements for athletes who participate in two sports at the same time at Indian Creek Middle School.

(6) - This is the 2010-11 IHSA/IESA Physical Form. This form must be filled out by a physician and turned into the athletic office or school office before any athlete is able to participate in a practice or game/event.
